Overview
This opportunity is for a Medical Coder specializing in risk adjustment and diagnosis coding. The role focuses on delivering accurate, compliant coding that supports healthcare program performance, audit readiness, and regulatory alignment.
About the Role
This position is dedicated to coding workflows, including reviewing clinical documentation, assigning diagnosis codes, validating records, and ensuring compliance with established coding standards. The role requires precision, consistency, and the ability to manage workload independently in a remote environment.
What You’ll Do
- Review medical records and assign accurate diagnosis codes in accordance with ICD-10-CM Official Guidelines and coding best practices
- Apply coding aligned with CMS-HCC risk adjustment models and related standards
- Provide clear and consistent documentation to support all coding decisions
- Ensure compliance with CMS requirements and maintain audit readiness at all times
- Maintain coding accuracy at or above 95% with a focus on continuous quality improvement
- Participate in audits, peer reviews, and internal quality checks
- Identify and flag incomplete or inconsistent documentation for further review
- Complete assigned workloads while meeting productivity targets (minimum 1,000 encounters per month)
- Meet deadlines to support monthly reporting cycles and operational timelines
- Utilize internal systems, tools, and dashboards to perform coding tasks efficiently
- Follow structured workflows and escalate documentation issues when needed
- Communicate coding-related questions and improvement opportunities to internal stakeholders
- Participate in team meetings to stay aligned with updates, processes, and standards
Qualifications
- Active certification required (CPC, CCS, RHIT, or CRC)
- 1–2 years of experience in medical coding, preferably in risk adjustment or HCC coding
- Strong understanding of CMS-HCC models, diagnosis coding guidelines, and RAF scoring
- High attention to detail with a strong focus on accuracy and compliance
- Ability to work independently in a remote, deadline-driven environment
- Experience with EMR/EHR systems and coding platforms
- Strong written communication skills for documenting coding rationale
