Program summary
As part of the DSNP care team, you will help evaluate member needs, address social determinants of health (SDoH), coordinate care, and close gaps in preventive health services. You’ll play an essential role as we expand DSNP services into new markets nationwide.
Key responsibilities
- Conduct annual Health Risk Surveys to support individual care plans
- Escalate newly identified health or safety risks to the care manager
- Complete delegated care coordination tasks within required timeframes
- Report quality-of-care concerns to the care manager or associate manager
- Advocate for members’ needs and provide solutions to address care barriers
- Use motivational interviewing to improve member engagement and promote wellness
- Document care coordination per regulatory and company guidelines
- Locate alternative contact methods for unreachable members

Required qualifications
- 2+ years of experience in behavioral health, social services, or related fields
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook, OneNote, Teams)
- Ability to work from a private, dedicated workspace
Preferred qualifications
- Experience in case management, discharge planning, or managed care
- Associate’s or Bachelor’s degree in behavioral health or human services preferred
- Non-licensed master’s level clinicians are also encouraged to apply
Education
- High School Diploma or equivalent required
- Relevant college education or equivalent experience preferred
Location: Franklin, Ohio
Pay: $65,000 – $80,000 per year
Schedule: Full-time, temporary contract role

A manufacturing organization in Franklin, OH is seeking an HR Generalist to support over 50–55 salaried employees and 200+ hourly employees. This contract role is ideal for a proactive and adaptable professional with strong HR knowledge and a hands-on approach to working in a plant environment.
Key responsibilities
- Manage daily HR operations within a manufacturing facility
- Act as the first point of contact for employee inquiries and issue resolution
- Maintain HR records, process documentation, and update the HRIS system
- Ensure compliance with federal, state, and local labor laws
- Support managers with performance reviews and employee development programs
Requirements
- Bachelor’s degree in HR, Business Administration, or related field (or equivalent experience)
- 2–4 years of HR experience — manufacturing or automotive industry preferred
- Solid understanding of HR policies and labor law compliance
- Strong communication and interpersonal skills
- Proficiency with HRIS systems and Microsoft Office Suite
